all students of the Urbana School District.
--Walk-ins welcome as time is available.
--Appointments are preferred.

School, Tiger Alley, next to Automotive, behind the gym doors.
Outside entrance only. Cannot access from inside the school.
Look for the Orange and Black awning which says Urbana Student
Parental Consent forms are required for most medical services.
Separate Consent form for dental required
Confidential Health Services as per Illinois State Law are
provided without parental consent.

Non-operative Gynecology Services
Substance Abuse counseling and treatment
Nutritional counseling and dietician support services

Currently provided: Preventive Services, cleanings and
Additional services in development: extractions and fillings
Dietician available on limited basis to work with children
on meal planning and calorie counting.

Evaluation and Counseling available by licensed social
workers and advanced practice nurse pracitioners
|Subtance Abuse Counseling and Treatment
Licensed counselers available by appointment to offer
confidential evaluation and treatment
