--Appointments are preferred.

Outside entrance only. Cannot access from inside the school.

Parental Consent forms are required for most medical services.
Separate Consent form for dental required
Confidential Health Services as per Illinois State Law are
provided without parental consent.

workers and advanced practice nurse pracitioners |
