The Urbana School-Based Health Center, located at 1002 S. Race Street, adjacent to Urbana High School, serves all children of the Urbana School District.

HOURS:
Medical, dental and mental health services
Monday through Friday 8 a.m.-4 p.m.

Services Provided:

Complete Medical Services
Physical Exams – School and Sport Physicals
Acute and chronic medical conditions: asthma, diabetes, ADHD, etc.
Gynecological exams, STD testing and treatment, pregnancy testing
Sport-related injuries and sub-specialty referral services

Dental Services
Screenings, cleanings, sealants, simple extractions, fillings
(Complex cases will be sent to CUPHDs main clinic on Neil)
Mouth guards for sports will be fitted and made on premises

Mental Health Services
Crisis intervention, depression, anger management
Individual and family counseling
Addictions counseling (individual and family)
Tobacco cessation

Nutrition Services
Individual and family dietary management, weight loss plans
Exercise prescriptions

Prevention
Behavioral Health Risk Assessment and Counseling
Individual and small group sessions

Coordination of Services
The Health Center works closely with school district social workers, counselors, teachers, administrators, juvenile justice system, primary care providers and other service organizations within the community.

Vision and Hearing Screenings

Vision and hearing screenings will be administered during the school year according to Illinois State Public Health guidelines. The mandate requires hearing screenings for all students in preschool, kindergarten, 1st, 2nd and 3rd grade, and all students in special education classes. Vision screenings are required for all preschool, kindergarten, 2nd grade, 8th grade and all students in special education classes. The screenings are conducted by the district nurses.

Vision screening is not a substitute for a complete vision and eye evaluation by an eye doctor.  Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an evaluation has been administered within the last 12 months. 

In addition to these students any student entering Urbana School District who has not previously been screened and any student referred for a special education case study will be screened for both vision and hearing. Any student suspected by a parent, guardian or teacher of having a vision or hearing problem may also be referred for testing. Please contact the district nurse’s office at 217-384-3564 if you wish to exempt your child or if you would like to know the date the screenings will be done at your child’s school.
